How to Book an Appointment

1. FILL OUT THE MONSTER MASH CLIENT FORM BELOW. 

Details are important - we want to make you look your best and give you the appropriate price. Include reference photos if you can - otherwise we will have to interpret your words to the best of our abilities!

2. An associate will call or email you to confirm details, make sure you are getting exactly what you want, quote you a rate, and put you in an appropriate time slot.

3. A 50%, non-refundable deposit is required to hold your appointment.

4. The other half of your deposit is due on or before the day of your application. Should you want extra services or prosthetics, an associate will help you on site to quote a final price.

Remember: don't procrastinate - our slots fill up quickly!
